3、投标人资格要求

投标人应具备的资格或业绩:1）境内企业须具有独立法人资格，提供有效营业执照；境外企业，根据该国（地区）的法律，须提供在主要制造厂地址注册的有关证明文件。

2）投标人须提供*开通会员可解锁*（以合同签订时间为准），不少于10台用于30万吨/年及以上高密度聚乙烯装置，容积不小于200m³聚合反应釜，且连续稳定运行2年以上聚合反应用搅拌器成功在用业绩。须提供合同扫描件及用户证明（合同至少包含首页、技术部分、签字页），如合同不能体现以上内容，须提供其他相关有效证明材料，原件备查。

3）投标人如为代理商，须提供HDPE搅拌器制造厂商的授权书，同一品牌同一型号产品只接受一家代理商参与投标，且不接受针对本项目出具的授权书(提供授权书扫描件)。同时代理商投标需提供代理商签订的代理产品销售合同一份。

for300,000t/aormorefromJanuary1,2013tothebiddingdeadline(subjecttotheContractsigning

time),whosevolumeshallnotbelessthan200m3polymerizationreactorandwhosemixerfor

polymerizationreactionhasbeenrunningcontinuouslyandsteadilyformorethan2yearsandisstillin

use.ThescannedcopyoftheContractandtheUserCertificateshallbeprovided(includingthefirst
